REASONS FOR REJECTION OF DOCUMENT BY PROCESSING OFFICER


25% / ENBLOC / NSITF PAYMENT

• Wrong signature on the notice form
• Account details stated on Notice Form different from bank confirmation submitted
• Date of disengagement is less than 4 months on the Resignation letter
• Status of remittance is not clearly stated on Evidence of Accrued Rights / Remittance Letter
• Name on Bank Confirmation not the same as the database
• Incomplete account number on Bank Confirmation Letter
• No bank stamp on the attached passport on Bank Confirmation
• Passport on NIN Slip not visible
• Expired Means of ID (Driver’s License or International Passport)
• Name on the Means of ID not the same as the database
• Wrong date of birth on Declaration of Age/ Birth Certificate
• Altered date on the declaration of age / Birth Certificate
• Written application Letter not signed
• Date of disengagement not captured on the affidavit
• Affidavit of Closed or non-functional organizations, stolen, burnt, misplaced disengagement letter should be clearly stated on the affidavit as the reason for having an Affidavit


DEATH BENEFIT


• Date on bank confirmation should not be less than six (6) months.
• Birth certificate if Minor is mentioned with a guardian on Letter of Administration
• Valid means of identification for NOK (driver’s license, National ID & international passport)
• Stamped passport affixed on bank confirmation.
• Date of death discrepancy on Introduction letter & Letter of administration against Death Cert.
• Endorsement of Introduction letter, Death cert & Letter of Administration.
• Confirmation report (Introduction letter, Death cert & Letter of Administration)
• Affidavit for name reconciliation, affixed with a passport stamped by the court.
• Change of name Doc (Newspaper Publication & Affidavit )
• Affidavit from NOK on the system, affixed with a passport stamped across by the court, when NOK is different.
• Legibility of documents submitted.
• All affidavits must have a passport affixed & stamped across by the court.
• In absence of Deceased ID, Notary Public Attestation of identity must be provided.
• Introduction letter must be provided for a non-retiree deceased.
• Notary public attestation is only acceptable for retirees.
• Bank confirmation letter affixed with a passport stamped across by the bank
• All NOKs must be signatories to Joint Bank
• Validation of Deceased Passport against database biometric

 

LUMPSUM PAYMENT

• Wrong pay slips, illegible payslip, and or inappropriate payslip
• Data recapture related issues (not yet recaptured or recaptured but with errors)
• Irregular signature on the retirement benefit forms


AVC PAYMENT


• Due to non-qualification of the client to access voluntary contribution (i.e if the client contributions are less than 24 months in the RSA statement or the withdrawal period is less than 2 years from the previous withdrawal  date)
• Tax Identification Number (TIN) not available
• Data recapture related issues(not yet recaptured or recaptured but with errors)
• No evidence of pension payment for retirees exempted from the CPS  
• No letter from embassy confirming relocation of foreigners


ANNUITY PAYMENT


• Where annuity agreement is not accompanied with a signed written application by the Annuitant.
• Where the RSA balance is less than the premium quoted.
• Where retiree moving on PW opts to move to annuity and has not stayed for 12 months on PW.
• Where there is Data recapture related issues(not yet recaptured or recaptured but with errors)
• Where there are irregular signatures on the document and on our database
